Ask for a copy in person

1. Download and fill out the following form:

• English version: Health release form
• Spanish version: Health release form
• Chinese version: Health release form

2. Bring the completed form and a valid photo ID to:

2. Find your clinic address on the last page of the form.

3. Mail your form to that clinic.

We will get back to you within 5 to 10 business days.

To pick up your records, bring a valid form of photo ID.
